Background
Antibiotics are used to treat bacterial infections. Antibiotics are usually administered orally. They are given either by mouth, or intravenously. However, they are used at different levels and forms and have different potential side effects, especially in the treatment of infections caused by certain types of bacteria. We evaluated the efficacy of intravenous intravenous antibiotics, as well as the potential side effects of intravenous antibiotics, in the treatment of patients with acute sinusitis.
Methods
This is a retrospective observational study using a prospective database of patients with sinusitis admitted to the University of Pennsylvania Hospital between January 2000 and March 2007. Patients were admitted to the hospital with an acute exacerbation of chronic bacterial sinusitis. Patients were treated with either a single dose of a single intravenous antibiotic (Zithromax®), or a combination of single and combination antibiotics (Zithromax® and Zmax®). All patients received a combination antibiotic at the same time of day. Patients were followed for a period of 4 weeks. The antibiotic was titrated to achieve bacteriological cure for each patient. Patients were followed for a period of 8 weeks. All patients had their prescribed antibiotic therapy for 1 week and then received intravenous antibiotics. A total of 1059 patients with acute sinusitis were included in the study. Overall, 909 patients (7.5%) had a total of 759 antibiotic treatment courses. The treatment courses were as follows: 1. A single antibiotic (Zithromax®) 500 mg; 2. A combination antibiotic (Zithromax® and Zmax®) 500 mg; 3. Zmax® 500 mg; and 4. Zithromax® 500 mg. All patients were treated with a single antibiotic (Zithromax®) for 8 weeks. The mean time from the start of the treatment to the final dose of the antibiotic was 9.7 ± 9.3 days. Overall, the antibiotic treatment courses were as follows: 1. Zmax® 500 mg. All patients had their antibiotic therapy for 1 week, but the course of treatment lasted for 8 weeks. The average duration of antibiotic treatment for patients who had a total of 759 antibiotic courses was 3.4 ± 3.4 days. The average time from the start of the antibiotic to the final dose of the antibiotic was 5.5 ± 2.8 days. The average duration of antibiotic treatment for patients who had a total of 759 antibiotic courses was 4.4 ± 3.4 days. The time to bacteriological cure for patients who had a total of 559 antibiotic courses was 3.3 ± 3.1 days. No significant difference was found between patients who were treated with an antibiotic of either Zmax® or Zmax® at all, and patients who were treated with either a combination antibiotic at all. In conclusion, intravenous antibiotics are effective in treating patients with sinusitis caused byStaphylococcus aureusand their sinusitis caused byStreptococcus pneumoniae.
Conclusions
Zithromax® is a safe and efficacious oral antibiotic that has no systemic or local side effects. The combination of Zmax® and Zmax® and a single antibiotic (Zmax®) is safe and well tolerated in patients with acute sinusitis caused byS. aureus.
